After completing the form you can:
1) print and mail to the appropriate school; or
2) attach and email to the appropriate school.
Additional Information:
1) Birth certificate, immunization record and two proofs of residency (showing your physical address) must be received prior to the first day of the 20/21 school year.
2) All screenings will take place when school opens in the fall.
